STEP A  
You MUST complete all of the on-line material prior to attending class and bring ALL of the following
printed materials along with the registration fee (checks made out to OSSRC)to the in-class clinic session.
1) Completed Data Sheet and ReCert Test Answer Sheet
2) Completed Module Presentations Quiz Answer Sheets (2)
3) Copy of your NFHS Concussion training certificate. The clinic administrators are required to verify concussion training is current by viewing a copy (paper preferred or electronic) of this training certificate. NOTE - If your concussion completion date is prior to Jan 1, 2016, you MUST redo concussion training. Print out the new concussion certificate and update concussion date and completion number in the OSSRC database. Bring a copy of the new certificate to registration.
4) Pre-Printed and Signed Referee Registration Form (No blank forms will be accepted)

ALL REFEREES MUST BRING THE CONCUSSION TRAINING CERTIFICATE WITH THEM.

5) Copy of your Risk Management Form for 2018 (18 and older)
No forms .. or .. No on-line print-out sheets ... or .. No fees = NO RE-REGISTRATION

If your are 18 or older, you must complete OSYSA Risk Management. 
Print the certificate and bring it to class.

RISK MANAGEMENT
  • Attendees need to pre-register prior to the training session in order to guarantee space.
  • Clinic sessions maybe subject to cancellation due to lack of registered participants.
  • Participants should check the OSSRC website for any last-minute changes.
